- Our membership plan is not insurance, a qualified health plan under the Affordable Care Act, pre-payment for future services or payment for access to discounted services. It is a direct payment arrangement for services, products and care provided.
- The membership plan includes a 30-day money-back guarantee. You may receive a full refund up to 30 days after purchase if no discounts or services have been used.
- Your payments must be current in order for you to receive the services and discounts included in your membership plan.
- Payment for treatments not included in your plan are due at the time of services.
- Membership plan benefits may not be combined with insurance or other offers and discounts.
- Membership plans do not cover procedures completed by specialists outside our practice.
- Membership plans run for 12 months and include an auto renewal feature. You may opt out of this auto renewal at any time, and you will receive an email reminder 45 days prior to the renewal date.
